8/4/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival to the home setting, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her afternoon. Ms. Broadnax verbalized to QMHPA that her afternoon was going well. QMHPA informed Ms. Broadnax that this session was going to focus on reducing her depressive symptoms. Upon arrival, she did not present any symptoms of S/I. Intervention: QMHPA began the session by inquiring about Ms. Broadnaxâs current thoughts, feelings and emotions. QMHPA challenged Ms. Broadnax to think about current symptoms, specifically her reported depressed mood, and possibly identify additional triggers to the symptoms. QMHPA listened to and validated Ms. Broadnaxâs response. QMHPA listened and observed as Ms. Broadnax began to process her current stressors and its effects on her current thought process and behaviors. QMHPA also assisted Ms. Broadnax with identifying several methods of addressing her depressive mood (to include meditating, deep breathing and listening to calming music). QMHPA praised Ms. Broadnax for candidly discussing such difficult issues and affirmed Ms. Broadnaxâs thoughts regarding her current stressors. Response: Ms. Broadnax appeared to be receptive to QMHPAâs interventions and services that were provided to her during the session. Ms. Broadnax reiterated her concerns with his daughter and other family members and identified her motherâs negative attitude towards her as a major trigger for her. She verbalized to QMHPA that a lot of her depressive symptoms could be easily avoided if she didnât have to deal with her family. Ms. Broadnax discussed with QMHPA that at times she just wishes that she could move far away so that she would not have to deal with them.
